Working together to improve care

We are proud to lead an exciting new initiative aimed at transforming palliative care for people experiencing homelessness. Across the UK, many individuals in this community face sudden, untimely, and undignified deaths, often without access to the care and support they need. Frailty, multiple long-term conditions, and unmet palliative care needs frequently go unrecognised, while homelessness services focus on recovery rather than deteriorating health or end of life care.

To tackle these challenges, we, in partnership with New Hope, have been selected as one of just 10 sites nationwide to participate in Phase 3 of the IMPROVE (Intervention to optiMise Palliative caRe for peOple with liVed Experience of homelessness) programme. This initiative brings together local professionals and organisations to form multidisciplinary, multi-agency Communities of Practice, sharing knowledge and exploring solutions to improve care for some of the most vulnerable members of our community.

The programme, delivered across eight sessions in 2026, will include:

  • Video presentations from experts and people with lived experience, followed by discussion
  • Opportunities for participants to bring real cases to inform collaborative problem-solving
  • A structured framework to share best practice and explore local system challenges
  • Access to practical resources, including guides for attendees
