Thinking and planning ahead

It’s natural to feel overwhelmed when thinking about the future – especially when it comes to planning for your care. We’re here to support you to reflect on what matters most to you. Recording your wishes in an Advance Care Plan can bring you – and those close to you - peace of mind and ensure your voice is heard.

One-to-one or group sessions

You can choose one-to-one support, or join our three-week group sessions on Advanced Care Planning—whichever suits you best.

We will help you explore key questions such as:

  • What matters most to me in life and care?
  • Where would I prefer to be cared for?
  • What treatments would I want – or not want?
  • Who do I trust to speak for me if I can’t? (e.g. lasting power of attorney)
  • How do I plan for emergencies?
  • What might care look like if I need more support?
  • How can I refuse treatments or make resuscitation decisions?
  • How do I make a Will?

Who can I bring? You can attend the course alone, or with a loved one.
