Who we care for
We support people living with progressive life-limiting illnesses, including cancer, non-malignant conditions and frailty.
Patients can receive our care from diagnosis, alongside active treatment. Our support is not only for people approaching the end of life.
Why refer early?
Early referral gives us more opportunity to support patients and those closest to them. It can help:
- Reduce crisis points and unnecessary hospital admissions
- Support patients to live well with their illness
- Support Advance Care Planning and ReSPECT conversations, saving healthcare professionals time and improving patient care
