We will help you explore key questions such as:
- What matters most to me in life and care?
- Where would I prefer to be cared for?
- What treatments would I want – or not want?
- Who do I trust to speak for me if I can’t? (e.g. lasting power of attorney)
- How do I plan for emergencies?
- What might care look like if I need more support?
- How can I refuse treatments or make resuscitation decisions?
- How do I make a Will?

An Advance Care Plan is a way to reflect on, discuss, and record what matters most to you - including the type of care you would want in the future. This allows you to stay in control of your choices at the end of life. Our Advance Care Plan template is available to download and can be completed either electronically or by hand.
