Helping you plan for your future
Our Advance Care Planning course helps you plan for your future.
The 3-week course covers what an Advance Care Plan is and why it is helpful to you, and those caring for you.
During the course, a qualified healthcare professional will guide you through conversations about lasting power of attorney, Wills, care options, preferred place of care and end of life care, resuscitation, and funeral plans.
In the final week, if you are ready and want to proceed with completing your Advance Care Plan, a qualified healthcare professional will help you do this. Once the document is completed, our team will email a copy to your GP, we keep a copy on our records and then you take home the original copy.
It is important to remember that your Advance Care Plan can be changed/updated at any point – it is not a legal document, but a recording of your wishes.
The course runs on a Wednesday at Grove House in St Albans and Peace Hospice in Watford, from 1pm to 3pm and is completely free.
A maximum of four patients attend the course and you can attend either on your own or with a loved one.
