A new way to contact us directly
People living with a progressive life-limiting illness can now access support more easily through a new online self-referral form from Rennie Grove Peace Hospice Care.
Our new Confidential Enquiry Self-Referral Form allows people to reach out for help as soon as they feel they need it, even from the point of diagnosis, without having to wait for a referral from a GP or other healthcare professional. Family members, friends and carers can also complete the form on someone’s behalf, with their consent.
For many people, navigating care options during an already difficult time can feel overwhelming. This new online option makes the first step towards getting support simpler and quicker.

As soon as a healthcare professional has confirmed a diagnosis, people can complete the online form and access the full range of our services. This includes early emotional support to help individuals and those around them come to terms with their diagnosis, as well as Living Well support in St Albans and Watford, which helps people build confidence and continue living as fully as possible. These services offer practical advice and supportive therapies tailored to individual needs.
As illness progresses, we deliver specialist end of life care, ensuring people receive the right care, in the right place, and in line with their wishes. After a death, bereavement support through groups, Compassionate Cafés and companionship visits is also available to help those around them navigate grief and find comfort.
While referrals from healthcare professionals will continue as usual, the new self-referral option adds flexibility and convenience.
The form is confidential, quick to complete and available 24/7.
