Would your patient benefit from our Outpatient Services?
We provide a range of day services at Grove House in St Albans, Hertfordshire and Peace Hospice in Watford. In addition, some outpatient services are available in the community across our catchment area of Bucks and West Herts.
All our outpatient services offer support with patients’ overall health and wellbeing. For example, clinics, therapies and courses offer practical and emotional support to help patients live well throughout their illness.
For full details of the support our outpatient services can provide, please see our outpatients services information page.
Our outpatient services are open to anyone who has been diagnosed with a life-limiting or potentially life-threatening condition, regardless of prognosis. For example, we can support patients who:
- have recently received a diagnosis
- are currently receiving treatment
- are living with a long-term life-limiting illness
- who have been successfully treated but need physical or emotional support to aid with recovery
Buckinghamshire patients are welcome to use our Grove House and Peace Hospice outpatient services, but we can also suggest equivalent services closer to home.
Want to know more about our services before you refer?
Contact our Coordination Centre to find out about our wide variety of over 40 services – for your newly diagnosed patients to those needing end-of-life care. We can help you find the services that best fit your patient’s needs.
Although patients can self-refer to our outpatient services, we will need to liaise with their GP before we can start providing care. For this reason, it is more efficient if their GP or another clinician involved in their care makes the referral.
To refer a patient registered with a GP in Hertfordshire:
All referrals go via a triage centre. Please download and complete a referral form choosing ‘Grove House, Rennie Grove Peace Hospice Care’ as the requested service and attaching any relevant documents. Contact information for submission can be found on the form.
Please do not refer without the patient’s consent.
