Improving palliative care for people with learning disabilities

A new multidisciplinary team meeting to better support people with a learning disability living in West Hertfordshire who need palliative care, was launched last month.

This new initiative will help improve shared care for patients with a learning disability and maximise learning across both the West Herts learning disabilities and palliative care teams.

The initial multidisciplinary team meetings have been held at each of the buildings across the area that patients attend: Peace Hospice in Watford, Grove House in St Albans and Hospice of St Francis in Berkhamsted.

This has allowed the learning disabilities and palliative care teams to see each location in person so they are able to let patients know what the venues are like, based on firsthand experience.

The multidisciplinary team meetings will be held monthly.
