FAQs
Our Compassionate Communities services provide community-based support to help beyond patient palliative, end-of-life care and bereavement needs. The support is from diagnosis and involves working in the community with local partners and volunteers to reach more people earlier.
The focus is on tackling the loneliness and isolation often experienced by people affected by bereavement and loss or those facing a life-limiting illness.

Our Compassionate Communities services are available to anyone who has been diagnosed with a potentially life-limiting illness and/or is bereaved.
We accept both professional and self-nominations for Compassionate Communities, as we recognise that people in emotional difficulty may need support in getting help. No formal nomination form is required.
Individuals can self-nominate to this service by making an enquiry using the email address or appropriate phone number for their area as given below.
If you are facilitating a nomination for a patient, their carer or family member, or for an adult in need of bereavement support, please ensure you have obtained their permission before contacting us.
