Our Compassionate Communities offers community-based support to help beyond a patient’s palliative, end-of-life care and bereavement needs. The support is from diagnosis, working with local partners and volunteers to reach more people earlier.
The focus is on tackling the loneliness and isolation often experienced by people who are facing a life-limiting illness or are affected by bereavement and loss.
The support is provided through the following services:
Within Bucks and Herts, Compassionate Communities services are available to anyone with a potentially life-limiting illness and/or bereaved.
