Referring to Compassionate Communities

Would your patient benefit from our Compassionate Communities service?

Our Compassionate Communities offers community-based support to help beyond a patient’s palliative, end-of-life care and bereavement needs. The support is from diagnosis, working with local partners and volunteers to reach more people earlier.​

The focus is on tackling the loneliness and isolation often experienced by people who are facing a life-limiting illness or are affected by bereavement and loss. 

The support is provided through the following services:

Within Bucks and Herts, Compassionate Communities services are available to anyone with a potentially life-limiting illness and/or bereaved.
