We’ve compiled a list of FAQs below to help


Our Compassionate Communities services provide community-based support to help beyond patient palliative, end-of-life care and bereavement needs. The support is from diagnosis and involves working in the community with local partners and volunteers to reach more people earlier.​

The focus is on tackling the loneliness and isolation often experienced by people affected by bereavement and loss or those facing a life-limiting illness. ​

Our Compassionate Communities services are available to anyone who has been diagnosed with a potentially life-limiting illness and/or is bereaved.​

We accept both professional and self-nominations for Compassionate Communities, as we recognise that people in emotional difficulty may need support in getting help. No formal nomination form is required.​

Individuals can self-nominate to this service by making an enquiry using the email address or appropriate phone number for their area as given below.​

If you are facilitating a nomination for a patient, their carer or family member, or for an adult in need of bereavement support, please ensure you have obtained their permission before contacting us.​
