Advance Care Plans – join us to find out what they are and why they are important

People with a life-limiting illness, and their loved ones, can find out more about how to plan for their future, at an event taking place at Watford General Hospital next month.

At a stall in the main foyer of Watford General Hospital on Wednesday 8 May, Rennie Grove Peace representatives, in partnership with Macmillan, will be available to chat with patients, and those close to them, about the benefits of Advance Care Planning.

The ‘What’s an Advance Care Plan?’ event is being held during Dying Matters Awareness Week which this year is encouraging people to think about the language they use to talk about death and dying.

The goal of Advance Care Planning is to help ensure that patients receive care that is consistent with their values, goals and preferences during serious and chronic illness and at end of life.

Alison May, Community Engagement Manager, says:

“At Rennie Grove Peace, we speak with people, and those close to them, about the value of planning ahead, from the moment someone is diagnosed.  Putting a plan in place that reflect someone’s personal values and life goals, that can be reviewed and changed throughout their journey, helps each person get the personalised care they want.  It also helps family members and carers to know what kind of care their loved one has chosen.”
